For a doctor with a DO licence
DO and MD are two types of doctor licence. For a DO, use Arizona's Osteopathic Board, the board that licenses DO doctors.
Use the Osteopathic Board's Find your DO search for this check. The Medical Board's records for medical doctors, or MDs, do not cover it.

For a nurse practitioner or physician assistant
A nurse with training for a further care role is called a nurse practitioner. In Arizona, that nurse can make care decisions without a doctor's approval, within their training. PA stands for physician assistant, a health worker who holds a separate PA licence.
Use the Nursing Board to check a nurse practitioner's nursing licence as well as their advanced-practice certificate. The certificate shows the nurse has permission for that further role in care. Check both records.
A nurse practitioner needs the Nursing Board's permission to prescribe, as of October 3, 2026, for patient groups covered by their training. Ask which kinds of patients the certificate covers and whether that includes your care needs.
For a physician assistant, or PA, use the Arizona PA Board's own search. It links to licence records.
Ask the Nursing Board whether the certificate shows a nurse-practitioner role and permission to prescribe for your care needs. For a PA, ask the PA Board which record confirms the person's role and permission to prescribe the medicine being offered.
The pharmacy needs its own check
Arizona's Pharmacy Board issues licences to pharmacists and permits to pharmacies, as of October 3, 2026. Check the pharmacy's permit.
To ship medicine into Arizona from another state, a pharmacy must hold an Arizona permit, as of October 3, 2026. It also needs permission where it operates.
The U.S. agency that reviews medicines is called the Food and Drug Administration, or FDA. Its pharmacy-check page leads to state records. A licence alone does not prove that a prescription is being filled properly.
Get the pharmacy's actual name. If the record is hard to find, ask the Pharmacy Board how to check that permit.
A blank history is not a full history
Arizona sets time limits for showing some board decisions about a licence on its website, as of October 6, 2026. Some decisions about licence problems or other concerns appear for no more than five years. Complaints still being looked into and dismissed complaints have separate limits on what can be shown.
Even if the online record shows nothing, a complaint or a board decision about the licence may have existed. Ask the board how to request more public records.
A public record with no problems listed does not tell you how good the care will be.
